The following services are available:

Skilled Nursing

Skilled nursing services are provided by an RN or LPN with training and experience in providing care in your home, including evaluation of needs and performing skilled nursing procedures like; wound care, catheter care, infusion services, medication management, and education of disease processes, self care techniques, coordination of care, and re-hospitalization prevention techniques. Your skilled nurse will help you and your family coordinate your home health care with your physician and other home care team members.

Physical Therapy

Physical therapy services are provided by a licensed physical therapist and a licensed therapy assistant under the direction of a physician.  Our physical therapists are trained in providing excellent therapy techniques in order to maximize and improve our patient’s functional ability. The plan of care is based on patient need and patient goals.

Occupational Therapy

Occupational therapy is provided by a licensed occupational therapist or a licensed assistant under the direction of the occupational therapist. Our occupational therapist will provide a comprehensive assessment to establish a treatment plan to maximize and improve our patient’s ability to perform everyday activities in the home. 

Speech Therapy

Speech and language therapy is provided by a licensed speech and language pathologist. Our speech therapist will provide a thorough evaluation of communication problems resulting from stroke, disease or illness, accidents, or developmental delays. Speech therapy can help patients improve cognitive function, improve swallowing ability, reduce risk of aspiration, and help to maintain adequate hydration and nutrition. Speech therapy can assist with adaptive speech devices, hearing rehab, non-oral communication, and education for patient and caregivers.

Palliative Care Consultations/Comfort Care

A palliative care consultation can assist the patient and family with clarifying goals of treatment and provide assistance with end of life decisions in your home.

Medical Social Services to assist with community resources, short term counseling, and financial planning, and assistance with living arrangements.

Home Health Aide services are delivered under the supervision of a register nurse or licensed therapist. Personal care assistance is on an intermittent basis. The certified nursing assistant facilitates the patient in returning to their previous level of independence with bathing and personal care.

Medicare/Medicare replacement Guidelines used to establish coverage

Medicare patients are required to be homebound as certified by a physician or have a qualifying illness or injury which requires assistance of skilled intermittent care by a RN, PT or ST therapist.
